ABSTRACT

Objective To investigate the health care-seeking behaviors of Mosuo and Pumi people.Methods The subjects were enrolled by using the multi-stage stratified random sampling method and surveyed by the self-designed questionnaire.Results To tally 1669 subjects including 1121 Mosuo people and 548 Pumi people completed the survey.When Mosuo and Pumi people suffer from ailments,they preferred to buy drugs in drugstores(47.3% for Mosuo and 46.9% for Pumi),followed by visiting a local township hospital(27.0% for Mosuo and 24.3% for Pumi).When they suffered from severe diseases,they preferred to visit the county/city/state hospital(93.4% for Mosuo and 91.1% for Pumi).The mental disease were mainly treated in the county/city/state hospitals(49.3% for Mosuo and 52.7% for Pumi);notably,39.3% of the Mosuo respondents and 31.5% of the Pumi respondents skipped this question.Conclusion Health education,including awareness-raising activities on mental health,should be enhanced in Mosuo and Pumi people to further improve their health care-seeking behaviors.

ABSTRACT

Objective·To investigate the information requirements of suicide attempters and their family caregivers, as well as the ability of family caregivers to take care of them. Methods·A total of 148 suspected suicide attempters from Department of Emergency of three hospitals in Shanghai and their family caregivers were enrolled in this study. Each group combined with one patient and one family caregiver. The patients and their family caregivers were interviewed by suicide module of the mini-international neuropsychiatric interview, homemade general information questionnaire, family caregiver task inventory, family adaptability and cohesion scale and information needs willingness questionnaire. Results·There were 84 cases (56.8%) of suicide attempters and 64 cases (43.2%) of non-suicide attempters according to the survey. Linear regression analysis showed that the family's ability of taking care of the patients is positively correlated with family intimacy and negatively correlated with the impact of such illness on the family (P=0.000, P=0.008).The scores of patients' information requirement about the status of disease, the treatment-related information, the way to obtain medical information, the cure probability, precautions after treatment, and self-care ability or support from family caregivers were higher than the expectation scores from their family caregivers. Equally, the scores of information requirement from the patients' family caregivers about above-mentioned information were also higher than the expectation scores from patients. Conclusion·The family care ability is related to the influence of the patients on the life of the family caregivers and family intimacy. For the patients and their family caregivers, there is a great demand for information about disease status and treatment situation, the way of seeking help, care information and so on. More information support and psychological assistance should be provided to suicide attempters and their family caregivers.

ABSTRACT

Objective·To explore the performance characteristics of the psychotherapy relationship in mindfulness-based cognitive therapy between the psychotherapists and patients with obsessive-compulsive disorder (OCD),and the strategies to promote the curative effect in mindfulness-based cognitive therapy.Methods·Three psychotherapists in Shanghai Mental Health Center and two groups of mindfulness-based cognitive therapy for OCD groups (a total of 11 people who completed treatment and 2 people dropped off) were selected.All the patients had mild to moderate OCD.The transcripts were transcribed into verbatim and coded using interpretative phenomenological analysis.Results·The psychotherapy relationship in subjective experience of patients with OCD showed warm emotional support,professional authority and individual requirements.The psychotherapy relationship in subjective experience of the therapist was manifested as equal companion,relatively mild and fuzzy distance.Strategies to promote efficacy from a therapeutic relationship perspective was profession to trust (especially for example),to promote emotional experience and to accept disappointment and negative emotions.Conclusion·Increasing the use of effective strategies can promote the establishment and positive development of therapeutic relationship in mindfulness-based cognitive therapy for OCD and promote the effectiveness of treatment.

ABSTRACT

Objective To study the prevalence and risk factors for personality disorder (PD)outpatients attending in for psychiatric and psychological counseling in Shanghai. Methods 3075 subjects were sampled by systematic sampling method from outpatients in psycho-counseling clinics and psychiatric clinics in Shanghai Mental Health Center. Based on DSM- Ⅳ criteria, personality disorders were assessed by both questionnaires (personality diagnostic questionnaire, PDQ-4+) and interviews (structured clinical interview for DSM- Ⅳ Axis Ⅱ , SCID- Ⅱ ). Logistic regression analysis was performed to determine the significant independent contributor to PD. Results 71.3% of the outpatients were found having pathological personality by using questionnaire of self rating PD scale. 982 outpatients (31.9%) met criteria for at least one personality disorder by using structured clinical interview. Younger age (OR= 1.8, 95% CI: 1.5-2.1 ), single or divorced (OR = 1.6, 95%CI:1.4-1.9), psychological counseling outpatients (OR=1.2, 95%CI: 1.1-1.3), mood and outpatients with neurosis disorders (OR = 1.7, 95%CI: 1.4-2.0) were more frequently assigned as personality disorders. Data from logistic regression analysis showed that patients of tender age, not nurtured and raised by their parents, with introvert characters were related risk factors of PD. Conclusion High prevalence rate of PD was found in this sample of Chinese outpatients, especially in thosepsychological counseling outpatients with mood or neurosis disorders. More attention should be paid to the recognition and intervention of PD in outpatients with mental disorders.

ABSTRACT

<p><b>AIM</b>To report the clinical experience during collecting sperm samples in the Fragile X syndrome (FXS) male patients.</p><p><b>METHODS</b>Two different polymerase chain reaction (PCR) based methods were used for the molecular diagnosis of FXS. Sperm collection was done mostly according to the laboratory manual of the World Health Organization.</p><p><b>RESULTS</b>We failed to collect sperm samples from five Fragile X subjects aged 18-60 years as a result of an unexpected erectile dysfunction (ED). Multiple examinations of the same subject at different times, and of different subjects from different provinces examined by different physicians, showed the same result consistently in all the five subjects.</p><p><b>CONCLUSION</b>Erectile reflex is an instinctive response in all healthy males. The absence of erection can be caused by hormonal, physical or neuronal malfunction. As hormonal profiles were reported to be generally normal in Fragile X men, we propose that an unknown physical factor or the neuronal circuit, or both, underlying the erection is compromised. The finding of this symptom in Fragile X patients may help better understand the clinical spectrum and pathogeneses of the disease.</p>
